Lesson Request

This form is used for BOTH GROUP AND PRIVATE lesson sign-ups. For information about levels, please visit www.stevensvillepool.com.

Do not fill out the availability section of this form! Private lessons will be scheduled individually, based on the package you selected. An instructor will contact you. Group lessons will be conducted according to the following schedule:

Level 1 (Age 3-4+) - 9:30am-10:00am

Level 2 (Age 4-5+) – 9:30am-10:00am

Level 3 (Age 5-6+) – 9:30am-10:00am

Payment will be taken at your first day of lessons. Stevensville City Pool accepts all major credit & debit cards as well as cash & checks. If you need to cancel your lesson, please do so with as much notice as possible. Aquatic classes issue prorated refunds if classes are unable to be conducted due to inclement weather. No refunds are issued if a makeup class is offered.
